 Terms & Conditions: Prior to ordering products all pets must attend one of our events and be examined to determine the appropriateness of any product ordered. You may only purchase products for the pet(s) that where examined at our event.

THE PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.

 

We take your privacy seriously, and we use reasonable business practices to protect confidential information of our clients. By using our website, online forms, online records, and prescription refills, you specifically agree and consent that we may use the Information and Communications in our sole discretion as long as such use is limited to our efforts to serve you and your animals. A $5 bio fee is added to the total of all vaccination sales for the proper disposal of the needle.

 

All locations and times are subject to change with or without notification.

 

A $10 transfer fee will be applied for all prescription transfers outside of ShotVet.

 

Thanks for Clearing the Shelters – Free Heartworm Test Program is for a limited time. Limit one per family. Proof of rescuing pet during national pandemic emergency is required.

An accurate weight is required for the dispensing of any and all prescription medications.

A Heartworm test is valid for 30 days following a negative test result unless the patient is actively receiving heartworm prevention.  After 30 days, another negative heartworm test may be required to dispense heartworm medication.

 

A patient doctor relationship is valid for 1 year from last visit.  In order to dispense any prescriptions, a valid patient doctor relationship must be established and up to date.

 

 

Home Care Instructions For Your Pet After Vaccinations

Possible vaccine side effects include: reduced or loss of appetite (lasting for one or two feedings), pain at the injection site, lethargy (lack of activity), reluctance to walk and/or run, and mild fever. Treatment is usually not indicated.

In case of emergency call your local emergency or full-service veterinarian; call immediately if you notice swollen face, hives, itching or vomiting, difficulty breathing, as that could be a sign of an allergic reaction to the vaccine.

Contact your local veterinarian in the event any physical and/or behavioral manifestations progressively worsen or continue beyond 2–3 days.

Pets, just like people, may run a fever, become sore, or have muscle aches after receiving vaccinations. Older animals which may have arthritis may seem more stiff & sore for a few days. These side effects are common and usually run their course within 24-48 hours without the need for further medical care. You may spoil your pet during this time and feed them their favorite food to get them to eat and drink. Do not judge your pet’s temperature by feeling your pet as that can be misleading.

Some normal parameters are listed below:

Cat normal temperature: 100-102.5 degrees F Cat normal heart rate: 100-120 adult, 130-140 young Cat normal respiratory rate: 20-30 breaths per min

Dog normal temperature: 99.5-102.5 degrees F Dog normal heart rate: 80-120 beats per min

Dog normal respiratory rate: 20-30 breaths per min

If your pet is on any other medication or is being treated for any other medical condition please contact your veterinarian before giving the medications listed below

● To the best of my knowledge, my pet(s) is healthy with no diagnosed allergies to vaccines and has had no recent occurrence of abnormal coughing, sneezing, vomiting, diarrhea or weight. My pet has not bitten or scratched anyone in the last 10 days. I have disclosed all medications my pet is on. I acknowledge the fact that a vaccine reaction is possible. Should it happen that my pet becomes ill due to a vaccination, I will not hold the Veterinarian or ShotVet responsible as reactions are highly individual immune responses which cannot be foreseen.

● A vaccine reaction is rare yet Potential side effects from vaccination include depression, muscle aches, stiffness, mild fever, or small lump at vaccine site. These side effects are usually temporary. Welts, facial swelling, vomiting, diarrhea, difficulty breathing, altered mental state, or shaking could be an allergic reaction. If any allergic reaction symptoms occur, proceed immediately to the nearest animal emergency facility.

 

 

FINANCIAL RESPONSIBILITY

● I understand that unforeseen side effects/allergic reactions may occur in any animal after the injection of a ShotVet act only to install these vaccines produced by major manufacturers of animal health products. Any side-effects/ allergic reactions requiring further medical attention is at my own expense. Due to the nature of the clinic setting, free medical for vaccine-related side-effects and allergic reactions during clinic hours will be given but is limited, and my pet may require further treatment at my expense. All other post-vaccinial events requiring treatment will be at my own expense. I have been given the opportunity to ask any questions concerning this policy before signing below. Being aware of these facts, I give my permission to the Veterinarian to administer the vaccines and medications listed on this document.

 

 

Media Release

I hereby grant and convey to Seminole Animal Hospital Services, LLC d/b/a Shot Vet and its authorized representatives all right, title and interest I may have in any and all photographs, video recordings, and other recording devices made during and around a visit to a Seminole Animal Hospital Services, LLC d/b/a Shot Vet clinic. I further agree that Seminole Animal Hospital Services, LLC d/b/a Shot Vet shall have the right to use such recordings for any and all purposes, an unlimited number of times and in perpetuity by any and all means and media, now and hereafter created.

 

YOUR PET HAS BEEN EXAMINED TO DETERMINE THE APPROPRIATENESS OF IMMUNIZATIONS SELECTED, THIS IS NOT A FULL AND COMPLETE PHYSICAL EXAMINATION.

 

I understand that as my pet’s healthcare provider, ShotVet may remind me via email, snail mail and/or text (standard text messaging rates may apply) when my pet(s) are due for refills, shots, and other medical related treatments.
